CONCIERGE MEDICINE

New patient - $400 (2 appointments)

First appointment: Includes obtaining history, discussing points of key concerns, reviewing previous labs/studies. Dr. Jadan will discuss her recommendations. 1 hour.

Second appontment: Check in after 2 weeks. She will cover areas of questions/concerns after you starting implementing the changes. 15 minutes.

Follow up appointments are time based:

Follow up appointment-$200. 30 minutes.
Follow up appointment-$100. 15 minutes.

There will not be a communication option in between visits. If you have a question, you must create a follow up appointment.

Acute visit - $100. 15 minutes.

MEMBERSHIP MODEL

$1000 membership fee to join. This is a yearly fee. Then thereafter, it will be $100/mo. What it includes: 2 appointments/mo up to one hour each for everything listed in the services tab. Unlimited communication during business hours via text message chat (8 AM - 8 PM M-F).

If a message is received after hours, it will be answered the next day.

*Upon activating your membership, you will have the opportunity to try the membership risk-free with the option to cancel your membership after the first month before you are locked in for a full year.
Contact Dr. Jadan to cancel any time during your one month cancellation period. Instead, you will be charged for the cost of the individual consult(s)
